Refer to the sponsor opportunity instructions to determine which narratives are appropriate for your submission.

  • At the present time, PHS/NIH requires all attachments in PDF format.
  • Not all types are appropriate with every submission. Only the Program Plan is mandatory for every submission.
  • Some types are conditionally required based on other aspects of your submission (e.g. Animal use requires Vertebrate Animals narrative). Failing to include conditionally required narratives may cause your submission to error at the sponsor.
  • Some types may not be appropriate for a particular opportunity, and if included, could cause errors at the sponsor.


These form-specific Proposal Attachment Types will appear in the Add Attachments > Attachment Type drop-down selection list after a valid S2S Opportunity is linked to the proposal that includes this form version.
